Teton Service Coordination’s team brings many years of experience and a variety of accumulated knowledge to this mission. We believe that when we support one another we provide the best quality of care to those we serve. Therefore, we strive to create an atmosphere of collaboration, in which every individual we serve can benefit from the combined knowledge and experience of our team.
Founded in 2017, Teton Service Coordination in partnership with Teton Supported Living, LLC has been serving our community for over 8 years. We have grown and adapted over the years to meet the changing needs of our community, while remaining true to our core values of compassion, respect, and integrity.
We offer Targeted Service Coordination (TSC) services for intellectually disabled adults. We provide case management, create individualized Individual Support Plans (ISP), and monitor care and services provided to our clients. We are committed to helping those we serve achieve their goals and improve their lives.

Qualifications:
• Minimum of a Bachelor’s degree in a human services field from a nationally accredited university or college and have twelve (12) months supervised work experience with the population being served; or
• Be a licensed registered nurse (RN) and have twelve (12) months work experience with the population being served.
Specific Duties:
• Link participants to developmental, vocational, mental health, and medical services, resources to meet basic needs, activities for social and leisure opportunities, and other services, resources, and activities that meet participant needs.
• Assist participants to retain access to services and resources.
• Link participants to services and resources to resolve a crisis, as needed.
• Collaborate with the participant and team to create an Individual Support Plan (ISP) that meets the needs of the participant to address functional, health, and safety needs.
• Monitor services identified in the ISP to ensure that services are rendered appropriately, that there is no duplication of services, and that participant needs are met.
• Monitor the participant's satisfaction with services, and meet face-to-face with the participant at least once every 90-days.
• Advocate for the needs and rights of participants.
